Individual Support:
Individual Support Services are “hands-on” individualized assistance with everyday activities that are required by an individual with severe and persistent mental illness in order to live independently in the community. The services are provided for adults ages eighteen (18) and older and are intended to support a person in a private home or licensed group home who has a documented transition plan and wants to move to independent housing. Individual Support can be provided while the person is living in the licensed facility or private home up to six (6) months prior to moving. Specifically, this service provides assistance with Instrumental Activities of Daily Living (IADL) including preparing meals, managing money, shopping for household necessities, using the telephone, housecleaning, laundry, transporting the individual to access the community, medication management, supervision and cuing. The goal is to provide coaching to the individual in areas of need and fade this support over time.


Peer Support:

Peer Support Services are structured and scheduled activities for adults age 18 and older with a diagnosis of Mental Health and Substance Abuse disorders and are provided by Peer Support staff. Peer Support Service is an individualized, recovery-focused service that allows individuals the opportunity to learn to manage their own recovery and advocacy process. Interventions of Peer Support staff serve to enhance the development of natural supports, as well as coping and self management skills. Interventions of Peer Support staff may also provide supportive services to assist an individual in community re-entry following hospitalization. Peer Support emphasizes personal safety, self worth, confidence, growth, connection to the community, boundary setting, planning, self advocacy, personal fulfillment, development of social supports, and effective communication skills. Services emphasize the acquisition, development, and expansion of rehabilitative skills needed to move forward in recovery.


Outpatient Therapy:
Outpatient therapy is the treatment of psychological, emotional, or behavior disorders through interpersonal communications between the patient and a trained counselor or therapist. The goal of many modern individual, family, and group therapies is to establish a central relationship of trust in which the client or patient can feel free to express personal thoughts and emotions and thus gain insight into his condition and generally share in the healing power of words. Such therapies include psychoanalysis and its variants, client-centered or non-directive psychotherapy, reality therapy, play and art therapy, and general counseling. Behavior therapy focuses on modifying behavior by reinforcement techniques without concerning itself with internal states.

 

Comprehensive Clinical Assessment:
A comprehensive clinical assessment is an intensive clinical and functional face-to-face evaluation of an individual’s presenting mental health, developmental disability, and/or substance abuse condition that results in the issuance of a written report, providing the clinical basis for the development of the Introductory Person Centered Plan (PCP), the Complete PCP, or the service plan when a PCP is not required. This written report also includes recommendations for services, supports, and/or treatment. Through a comprehensive clinical assessment, the information is essential to formulating a diagnosis and a plan for treatment is composed. The comprehensive clinical assessment provides the Qualified Professional and treatment team members with the necessary data and recommendations to synthesize this information into the development of the PCP with the consumer. A comprehensive clinical assessment offers an opinion as to whether the consumer is appropriate for and can benefit from mental health, and/or substance abuse services. It also evaluates the consumer’s level of readiness and motivation to engage in treatment. For individuals with substance abuse conditions, recommendations on the appropriate level of placement are determined using the American Society of Addiction Medicine (ASAM) Criteria.
