  • Copy of valid NC Driver’s License

  • Copy of auto insurance policy

  • Copy of driver’s record

  • Documentation of TB test (within the past year)

  • First Aid/CPR certificate

  • Blood-borne Pathogens/Infectious Disease certificate

  • Copy of Professional License (if applicable)

  • NCI certificate

Candidates for therapy can be fully or provisionally licensed in the state of North Carolina. Qualified Professional candidates must have at least two years post-graduate mental health or substance abuse experience with children or adults.
